Application to register

keyboardstethIf you wish to register you will need to complete the forms enclosed in the registration pack. This includes: personal details e.g. name, address, D.o.B. etc. and also a medical questionnaire to help us look after you whilst we are awaiting your complete medical record.

Registration Process

Registration forms should be fully completed making sure that:

  • Your name and address are legible.
  • You include your date and place of birth.
  • You provide up to date contact telephone numbers.
  • Tell us details about your current/previous GP.
  • Tell us if you come from abroad and provide us with the date of entry into England.
  • Provide proof of ID & Address (See below)
  • Sign the application.

NB.

  • You should complete your application for registering with ‘Oakview’ and the patient’s questionnaire as soon as possible.
  • Reception staff will ask you to provide a valid Photo ID & Proof of Address.
  • Normally the registration process takes 10-14 days approx.
  • Should you have an urgent medical problem we will normally see you as a temporary resident.
  • Once your forms have been processed we will contact you to make your screening appointment with the practice nurse. (You will be asked to provide a urine sample; a bottle can be obtained from reception).
  • Failure to attend a screening appointment with the nurse will delay the process of registration and may result in your application being unsuccessful.
  • Children under 6 do not need to see the nurse but we do need details of their medical problems and vaccinations in order to look after them properly.
  • Registration requires satisfactory completion of all these stages and you will not be able to access the regular services at the practice until this is finalized.


Registration Medical with practice nurse

Once your forms have been processed, we will contact you to make an appointment with the practice nurse. This is an essential part of registration and failure to attend will delay registration and may result in your application being unsuccessful. You will be asked to provide a urine sample, a specimen bottle can be obtained from reception.

Children under six years of age, do not need to see the practice nurse, but we do need details of any medical problems and their vaccinations.
